Feline lower urinary tract disease (FLUTD) is an umbrella term encompassing feline idiopathic cystitis (FIC, ~60-65% of cases), urolithiasis (struvite, calcium oxalate), urethral plugs, bacterial UTI (uncommon in young cats), and rarer neoplasia/anatomic anomalies. FIC pathophysiology involves a defective glycosaminoglycan (GAG) layer with neurogenic inflammation driven by sympathetic outflow in response to environmental stress, producing visceral hyperalgesia, urothelial leak, and recurrent cystitis without infection. Urethral obstruction — almost exclusively in males due to narrow distal urethra — causes rapid post-renal azotemia, life-threatening hyperkalemia, metabolic acidosis, and bladder atony or rupture within 24-48 hours if untreated.

尿道完全阻塞為急診，超過 24-48 小時可致命。阻塞後需監測阻塞後利尿 (post-obstructive diuresis)。

分診急迫度
次急迫・近期門診穩定但需注意惡化徵兆，建議安排近期門診 (stable but monitor for deterioration, schedule near-term visit)
監測重點
非阻塞性 FIC 通常 5-7 天內自限，但約 50% 會在 1 年內復發。環境豐富化可降低復發率。尿道阻塞及時解除後預後良好，但再阻塞率約 15-40%。PU 手術可顯著降低再阻塞風險。
